For Clinicians in our Communities

Welcome. Kaiser Permanente has a long-term commitment to strengthening the community health care safety net.

For many years we have worked closely with community health centers to improve the quality and cost-effectiveness of care, to help build effective clinic management infrastructure, and to collaborate on projects that reduce health disparities and promote a community-based system of disease prevention and management. In addition, in recent years, we have expanded and deepened these safety net partnerships by reaching out to local health departments and public hospitals.

In that spirit, we are including in this section of our web site, Kaiser Permanente tools and materials that we believe may be of value to our community partners.

We encourage you to check back periodically as we continue to expand this section of the web site.

Clinical Practice Guidelines
While these Clinical Practice Guidelines are specifically designed for health care professionals within the Kaiser Permanente Medical Care Program, we believe they may be of value as a reference for community clinicians.

Caring for Your Family’s Health: A Guide for African Americans
Eat nutritious foods. Exercise. Know your family history and "numbers." See a doctor routinely. This is the Rx at the heart of Kaiser Permanente’s African American Family Health Project supporting black Americans to boost their chances of maintaining or improving their health. Developed in collaboration with the American Academy of Family Physicians and the Institute of Church Administration and Management, “Caring for Your Family’s Health: A Guide for African Americans” is full of helpful information, charts, and space for each person to jot down details of their own and family members' medical histories. We'll soon be launching similar, concentrated efforts for Asians and Pacific Islanders, and then Latinos.
