For Clinicians in our Communities
Welcome. Kaiser Permanente has a long-term commitment to strengthening
the community health care safety net.
For
many years we have worked closely with community health centers to improve
the quality and cost-effectiveness of care, to help build effective
clinic management infrastructure, and to collaborate on projects that
reduce health disparities and promote a community-based system of disease
prevention and management. In addition, in recent years, we have expanded
and deepened these safety net partnerships by reaching out to local
health departments and public hospitals.
In that spirit, we are including in this section of our web site, Kaiser
Permanente tools and materials that we believe may be of value to our
community partners.
We encourage you to check back periodically as we continue to expand
this section of the web site.

Handbook (English)

Written as a health guide for our members and patients, this handbook
contains over 200 common health topics online. Learn more about prevention,
home treatment , healthy lifestyle choices and how to prepare for appointments
and communicate effectively with your healthcare team.

Health Topics (English/Español)
A health resource for you and your patients, our Featured Health Topics
brings together a wide variety of online resources—all focused
on a specific condition or health concern, and all collected in one
convenient place.
For each topic, we've carefully organized the most important information.
Each includes consumer health information, details about medications
and treatment options, and practical steps for managing a condition.
Many of the health topics are available in Spanish as well as English.
Topics include: Allergies, arthritis, asthma, child/teen health, colds/flu,
depression, diabetes, fitness, heart health, making health decisions,
mind/body health, nutrition, pain management, pregnancy/new baby, preventive
care, and weight management.
Drug
Encyclopedia (English/Español)
Intended for health care consumers, the encyclopedia can be searched
by drug name or by condition. Information is available in both English
and Spanish and includes access to printer-friendly format.
Caring for Your Family’s
Health: A Guide for African Americans
Eat nutritious foods. Exercise. Know your family history and "numbers."
See a doctor routinely. This is the Rx at the heart of Kaiser Permanente’s
African American Family Health Project supporting black Americans to
boost their chances of maintaining or improving their health. Developed
in collaboration with the American Academy of Family Physicians and
the Institute of Church Administration and Management, “Caring
for Your Family’s Health: A Guide for African Americans”
is full of helpful information, charts, and space for each person to
jot down details of their own and family members' medical histories.
We'll soon be launching similar, concentrated efforts for Asians and
Pacific Islanders, and then Latinos.
