Transition Plan

Created in 1990, the Transition Plan offers coverage for low-income individuals. The program offers transitional help for those who are in financial need, but are not eligible for programs such as Medicaid and Medicare.

Participants in the Transition Plan are referred through community partners and may be new or former Kaiser Permanente members.


Access to Care Program

Access to Care is a community-based program developed by the Healthy Connections Network in Akron, Ohio. The goal of Access to Care is to provide a “medical home” to patients who are without health insurance but ineligible for other assistance programs. Patients who have a medical home are likely to receive a better continuity of care are less likely to use high-cost emergency care services.

The program involves a network of Akron-area physicians who have agreed to give charity care to Access to Care patients. By 2007, eight Kaiser Permanente Physicians will be part of this network offering charity care.


Medical Financial Assistance Program

The Medical Financial Assistance Program is designed to provide financial relief for both members and nonmembers who are not able to pay for medical services. The program supports Kaiser Permanente's commitment to contributing to the overall health of the community by providing care at discounted rates to uninsured nonmembers and providing some financial assistance to eligible low-income members who have difficulty paying for health care services.
