CHRISTOPHER SPEVAK, MD, was, at the time of publication, an anesthesiologist, and the Medical Director of the Pain and Palliative Medicine Clinic for the Ohio Permanente Medical Group. His interests include health services policy and research.
	MARYANN DZVREC, PharmD, is a Clinical Information Specialist in the Kaiser Permanente Central East Division Ohio Market Office of Drug Information and Clinical Pharmacy Services. A graduate of Ohio State University, she currently serves as the Clinical Pharmacist for the Pain Clinic.
	RONALD COPELAND, MD, FACS, is the Medical Director for the Ohio Permanente Medical Group. Prior to assuming his current role a year ago, he served as the Vice President and Associate Medical Director.
